Our model of care and support provision is an approach that is underpinned by the ‘social model’ of disability. The approach focuses on the individual’s abilities, while providing support to meet needs resulting from the person’s disabilities or long-term health conditions. It is an evidence-based holistic approach to the provision and delivery of effective care and support services that empower adults to live active and fulfilling lives as valued citizens with valued roles in their communities.
At its heart, the model comprises of:

For us, person-centred active care and support is not an end in itself. Rather, it is a journey of self-discovery, continuous learning, development, and improvement to care and support for individuals to live the lives they have choose in a safe and effective way.
We know that the quality of the service we provide is dependent on the quality of the people we have delivering the care and support. This is why we invest in the training and development of our staff teams, pay people the national living wage, as well as provide ongoing support and development so that our teams remain knowledgeable and confident to deliver the objectives of our Statements of Purpose to the highest standard.
